It is not strictly required, but if you have it—or if you take chronic prescriptions—bringing your medication bottles and paperwork will help the clinical provider understand your medical history and guide your visit.
If the clinical evaluation determines that you clinically need recovery days, we will provide an excuse note as part of your visit.
The sports league will generally provide a blank Sports Physical Form. Bring it printed so our professional can complete and sign it after the evaluation. If you do not have one, let reception know.
